The Opportunity

The CareComplete suite of programs work in partnership with general practitioners and deliver a range of integrated services utilising health coaching methodology to provide an additional, coordinated support system to patients with complex health needs. We are looking for a Clinical Team Leader in Western Australia to manage a team of clinical and non-clinical staff in the delivery of CareComplete, every day you will enhance, improve and impact the health outcomes of program participants. Through inspiring leadership and mentoring, you will guide and empower your team to achieve improved patient health outcomes, an enhanced patient experience and reduction in overall cost outlays.

Key responsibilities will include:

  • Driving regional achievement of CareComplete objectives;
  • Identifying and acting on opportunities to improve program and team performance and contributing to the continual review of program elements;
  • Implementing project plans and liaising with relevant stakeholders to launch new products/programs, as required, while supporting and training team members through the roll out of new projects;
  • Providing inputs into design and refining the optimal workflow across the teams to ensure the most effective clinical and operational delivery models; and
  • Fostering positive working relationships with internal and external staff at all levels, and assisting in the development and success of team members.

About You

To be successful in this role, you will be an allied health professional or Div. 1 Registered Nurse, with the appropriate professional body registration, and a passion for leading teams and assisting people who will benefit from integrated care planning.

You will have experience in working with health programs focused on achieving multiple objectives, within primary and other community care environments. This includes knowledge of chronic disease management, discharge/care planning and/or post-discharge service provision to clients with complex co-morbidities.

Strong leadership and management experience, including the ability to manage staff working across multiple locations, and the ability to build relationships with internal and external stakeholders will be key to your success in this role. While not mandatory, a relevant post graduate qualification in Health or Management will be highly regarded.
